Sermorelin and HGH: Understanding the Difference

Sermorelin and HGH — understanding the difference

As interest in hormone optimization has grown, so has curiosity about human growth hormone (HGH) and peptides such as sermorelin. Both come up in conversations about body composition, energy, sleep, and recovery, but they work through fundamentally different mechanisms, and the distinction matters.

At Slender Me Medical we offer compounded sermorelin through a medically supervised telehealth program. Here is an overview of how the two differ.

What is HGH?

Human growth hormone is produced by the pituitary gland. It plays a central role in growth during childhood and continues to influence body composition, metabolism, bone density, and recovery in adulthood. HGH levels decline with age, and that decline has been associated with changes in body fat, muscle mass, and energy.

Synthetic HGH (somatropin) is a prescription medication that directly supplements the body's own growth hormone. It is FDA-approved for specific medical conditions. Use outside those approved indications is off-label and is subject to particular legal restrictions in the United States.

What is sermorelin?

Sermorelin is a peptide that mimics growth hormone releasing hormone (GHRH) — the signal the body uses to prompt the pituitary gland to release growth hormone. Rather than introducing growth hormone directly, sermorelin acts on the pituitary to stimulate the body's own release.

That is the mechanistic difference: sermorelin acts upstream, within the body's existing signaling pathway, while HGH acts downstream by supplying the hormone itself.

How they differ

Mechanism: HGH supplements growth hormone directly. Sermorelin stimulates the pituitary to release the body's own.

Regulatory status: Somatropin is FDA-approved for defined indications. Sermorelin as used in this context is a compounded preparation, which is not reviewed by the FDA for safety, effectiveness, or manufacturing quality before being dispensed.

Feedback regulation: Because sermorelin acts through the pituitary, the body's own regulatory feedback loops remain part of the process. This is a description of mechanism, not a claim of superior safety or outcomes.

Timeline: Neither produces immediate results. Any changes develop over weeks to months.

What the evidence shows

Sermorelin has a long clinical history, originally used in diagnostic testing of pituitary function and in pediatric growth hormone deficiency. Its use in adult hormone optimization is off-label, and the evidence base for that use is considerably more limited than for its approved indications.

Patients in our program report varying experiences with sleep quality, energy, and recovery. Individual response differs substantially with age, health status, lifestyle, and adherence, and reported experience is not the same as demonstrated efficacy.

How our program works

Sermorelin is available via telehealth to qualifying patients. A licensed provider reviews your health history, determines eligibility, and develops a treatment plan. Compounded sermorelin is dispensed by a licensed compounding pharmacy and shipped to you. Not all patients will qualify.
